How to Access Equipment
To obtain equipment, an assessment is required. You can request a referral from your GP or get in touch with your local council’s social care team.

Apply for help from the Wheelchair Service:
If you have a wheelchair to support your independence you, your next of kin, a care home or GP can contact us on their behalf and ask for a further assessment if your needs have changed significantly.
If you are not already working with the wheelchair service and you meet the eligibility criteria, a referral can be made by your GP, community nurse or allied health or social care professional.
